  • Option 1: Call 1-866-ARALAST

    Speak with an enrollment specialist who can answer your questions, enroll your patients, and connect you with all AATmosphere Program resources.

  • Option 2: Download a Referral Form

    To prescribe ARALAST NP therapy for your patients, fill out the Referral Form and fax it to 1-800-752-0595. An AATmosphere Program Reimbursement Specialist will verify benefits and get back to your office with results within 2 business days.

  • Option 3: Download an Enrollment Form for you and your patients

    To enroll in the AATmosphere Program, complete the Enrollment Form and mail or fax it to 1-800-752-0595. An AATmosphere Program Enrollment Specialist will work with you and your patient to make sure that you receive all the services you need.
